What questions should a service business ask on an enquiry form?

The right questions help you understand what a potential customer needs and decide how to respond. Keep the form focused: ask for information that changes your next step, and save detailed discovery questions for the conversation.

Start with the essentials

Most forms need a way to address the person, a reply method, and a description or category for the enquiry. If you provide several services, a short “What do you need help with?” selection can route the request. If you only provide one service, asking that may add little value.

Make it clear which fields are required. Email may be enough for a non-urgent request; phone may be appropriate when a call is part of the service. Avoid requiring both unless there is a clear reason.

Ask one useful context question

A short open question such as “Tell us a little about your project” can capture details that fixed fields miss. Give a helpful prompt, but don’t require a long essay. You can also use a few options to make answering faster, such as “I’m looking for: a quote / advice / an appointment / something else.”

Add qualifiers selectively

Some businesses need information such as project location, approximate timing, property type, or service size before they can respond. Add a qualifier only if it helps determine availability, fit, or the next action. Explain why you ask if the reason isn’t obvious.

Budget questions can help tailor a recommendation, but asking for an exact figure too early may discourage visitors. Consider optional ranges and include “I’m not sure yet.” Never collect sensitive details without a clear business need and an appropriate way to protect them.

Review your questions

Look at recent enquiries and ask whether each field helped you route or answer the request. If a question is often skipped, misunderstood, or never used, rewrite it or remove it. Test the form on mobile and ask a colleague unfamiliar with the process to complete it.
